13. Please confirm the consent to the provision of emergency first aid treatment by a trained RFC first aider for any injury sustained as a result of football.*
14. Please confirm permission for RFC to arrange the transportation of me/my child to the nearest hospital by ambulance, if required*
15. Please confirm consent to having my or my child's photograph taken during matches/training/events in connection with RFC. Pictures may be used on club marketing including the club website and social media.*
